Type locality: "Cordillera de Vilcabamba, CI/RAP Expedition Camp Two (11° 33´ 35 S, 73° 38´ 28W), 2,050 m, Provincia de Satipo, Departamento de Junín, Peru".

Holotype: AMNH 153049.
