Holotype: QCAZ 41612, by original designation.

Type locality: Cordillera del Cóndor, Miazi Alto (04.25656 S, 78.622456 W; 1300–1315 m.a.s.l.), Cantón El Pangui, Provincia Zamora Chinchipe, Ecuador.
