Holotype: MUSM 35879. Paratypes: ANSP 182815 (1); AUM 58781 (1); ROM 93945 (1).
Type locality: Peru, Padre Abad Province, Irazola District, San Alejandro River, Ucayali River drainage, 195 m a.s.l., 8°55'02"S, 75°12'26"W.
Etymology: The specific epithet albivermis is a combination of the Latin albus (= white), and vermis (= worm), and is in reference to this species’ variable but distinctive white to yellow markings.
