- Holotype: MHNSM 18653, adult ♂, collected by E. Lehr on 8 February 2003.
- Type locality: “Cordillera de Carpish, vicinity of Caserío Carpish de Mayobamba (09°43′50″ S, 76°06′46″ W; 2625 m), Departamento Huánuco, Provincia Huánuco, Peru”.
