- Holotype: ICN 33238, adult ♀, collected by Pedro Ruiz-Carranza, Mariela Osorno, Ruth Amanda Estupiñan, and Ricardo Sánchez on 22 March 1993.
- Type locality: “Hacienda El Tambito-Fundación Proselva, Inspección de Policía 20 de Julio, Municipio de El Tambo, Departamento de Cauca, Colombia (02°32′10.9″N, 76°58′04.2″W, 1,700 m asl)”.
