- Holotype: FMNH 69838 (field no. 5054), adult ♀, skin, skull and postcranial skeleton, collected by Philip Hershkovitz on 16 June 1950.
- Type locality: “9 km S Valdivia [Valdivia at 7°11' N, 75°27'W], Antioquia, Colombia, 1400 m”.
