- Holotype: LSUMZ 27167, adult ♂, collected by Richard Thomas on 28 November 1974.
- Type locality: “33 km (by road) SW Huancabamba, elevation approximately 7000 ft (~2133 m) (west of Cruz Blanca; below summit of Cordillera Huancabamba), Departamento Piura, Peru”.
