Type locality: Ainoshima Formation, Ashiya Group; "Southern end of Ainoshima Island, Kitakyushu City, Fukuoka Prefecture, Kyushu, Japan".
Holotype: KMNH VP 200,006
Etymology: (Latin) hexeris = "a vessel propelled by six banks of oars, in allusion to the large size of the species and its means of propulsion" [Olson & Hasegawa, 1996: 745]
