  • Holotype: UMMZ 159014.
  • Paratypes: UMMZ 157585 (2), 159013 (3).

Type locality: South Fork of Holston River, about 0.5 miles above South Holston Dam, Sullivan County, Tennessee, U.S.A., elevation 1490 feet.
