- Holotype: KU 173224, adult ♂, collected by David C. Cannatella and William E. Duellman on 10 January 1977.
- Type locality: “Pistipata, Río Umasbamba (=Río Santa María), 12 km SE Huyro, 1820 m, Departamento Cuzco, Peru (72°30′W, 13°03′S)”.
