- Holotype: MUSM 25345 (sample code ML 1352), adult ♂, collected by M. Lundberg on 2 November 2005.
- Type locality: “the mountain ridge close to the radio tower at Chacos (10.658S, 75.298W; WGS84), 2780 m a.s.l., Distrito Oxapampa, Provincia Oxapampa, Region Pasco, Peru”.
