Holotype: BPBM 39162.

Paratypes: AM I.44590-001 (1); ASIZP 69356 (1); BMNH uncat, (1); BPBM 31052 (1), 39163 (1), 39232 (2); LACM 56782-1 (1); USNM 392613 (1).

Type locality: West side of Motu Tautara, Eastern Island, depth 10 meters.
